Repair Order Form
Use this form for repair of orthotics that need refurbishment, we repair our brand orthotics or from other labs as long a medical practitioner who has an account with us is ordering for their patients.
Use for adjustments or modifications needed. Our orthotics carry free of charge adjustments for upto 6 months from the date of invoice.
As is specified on the warranty card attached to new orthotics patients have upto 3 months to follow up with you after orthotics are dispensed, this would give enough time for order any adjustments.
after 6 months any repair or adjustment would be charge accordingly to a flat charge repair,
